Job summary

Job description
Overview

We have an exciting opportunity for rehab focused Occupational Therapists to join the Community Division in the Wigan borough. This role is part of the new Intermediate Care at Home Team.

Responsibilities
  • Complete functional and specialist OT assessments of patients in community settings to facilitate earlier discharge.
  • Plan and implement effective interventions, working closely with community services while managing risk.
  • Provide an advisory service to patients, relatives, healthcare professionals and social care.
  • Assist the Team Leader in planning, coordinating, delivering and evaluating the intermediate service day‑to‑day.
  • Function as an effective member of the multi‑disciplinary team and integrated team within Health and Social Care.
  • Manage a defined caseload using evidence‑based/client‑centred principles to access, plan, implement and evaluate interventions.
  • Regularly supervise students on practice placement and provide leadership and clinical supervision for other junior staff as delegated.
  • Support team leads in the development and delivery of in‑service training for MDT colleagues.
  • Support service development alongside senior colleagues, including data collection, audits, service review and other duties incidental to the post.
Qualifications
  • Qualified occupational therapist with experience in community rehabilitation.
  • Experience in intermediate care at home or similar setting.
  • Excellent communication skills and ability to work as an effective team member.
  • Ability to independently travel across the borough to make multiple home visits each day.
  • Enhanced DBS with Adults' Barred List required (obtained during pre‑employment checks).
Working pattern

Monday to Sunday. Requires travel across the borough for home visits.
