Craft personalized care plans in collaboration with residents, families, and healthcare professionals. Ensure accuracy of care documentation and adherence to legal and team understandability. Lead with confidence by organizing the shift and guiding the team. Facilitate seamless transitions with clear shift handovers. Monitor health through routine observations and manage specific health needs with attention to detail and safety. Administer medications following MAR sheets and protocols. Build and maintain strong, trusting relationships with residents, families, and other healthcare professionals, addressing concerns with empathy and urgency. Promote independence by supporting daily activities while providing hands-on assistance as needed. Act on feedback to prioritise resident health and well-being. Uphold dignity and individuality, advocating for residentsul needs and preferences. Encourage resident engagement through social interactions and activities. Prioritise safety and maintain a secure environment. Support nutrition by monitoring for weight changes or dehydration. Respond to emergencies quickly, using the on-call system and reflective practices to improve care.
