Responsibilities

This opportunity of Reablement Support Worker offers a role working as a member of the Multi-Disciplinary Team (RRRT) under the guidance, and frequently without direct supervision, of the Therapists and Nurses, contributing to an effective rapid response and reablement service. The work includes assessments of clients and the provision of reablement therapy, monitoring, assisting with continuous healthcare checklists, nursing interventions and support to patients within their own home.

Qualifications

Essentially you will have RQF Level 4 - or Level 3 plus equivalent experience that is evidence based with a competency portfolio,.

Transport

Own transport is essential.
