Rate of Pay: £14.09 - £18.42 per hour

The Reablement Care Worker is a vital frontline role within the Cardiff Council's Community Resource Team (CRT) Homecare service, working in partnership with Cardiff and Vale University Health Board. The purpose of this role is to provide short-term, intensive, and outcome-focused support to service users in their own homes, typically following an illness, injury, or hospital stay. The core objective is to enable citizens to learn or re-learn the necessary skills for daily living, maximizing their functional independence and supporting them to achieve their wellbeing goals. This service is crucial for preventing unnecessary hospital admissions, facilitating timely discharges, and delaying or reducing the need for longer-term social care services.
