What you will do:

  • Undertake assessments and reviews, commissioning services in line with departmental policies and the Care Act 2014.
  • Co-ordinate safeguarding adults’ procedures, leading investigations and enquiries when necessary.
  • Collaborate with other teams, providers, and external agencies to deliver outcome-focused services for individuals and their families.
  • Empower individuals to make informed choices by complying with the Care Act 2014 and departmental policies.
  • Keep records and decisions up-to-date with the Department’s IT facilities.
  • Participate actively in professional supervision and staff meetings to continually improve practice.
