Job summary

A local authority care service in Upton is looking for an experienced Social Worker to support adults and carers with complex needs. This role involves conducting assessments, care planning, and safeguarding interventions, while working collaboratively within a multi-disciplinary team. Their teams work collaboratively to embed the principles of the Care Act 2014, ensuring personalisation, inclusion, and safeguarding are at the heart of service delivery. As an experienced Social Worker, you will support adults and carers with complex needs, delivering assessments, care planning, and safeguarding interventions. You will work as part of a multi-disciplinary team, ensuring positive outcomes and promoting independence, resilience, and wellbeing.

Key Responsibilities
  • Carry out needs-led assessments, care and support planning, and reviews.
  • Undertake safeguarding enquiries in line with the Care Act 2014 and interagency policies.
  • Provide social work support to adults and carers, involving them in planning and decision-making.
  • Work collaboratively with multi-disciplinary teams to prevent hospital admissions and promote independence.
  • Organise and chair multi-disciplinary meetings, case conferences, and strategy discussions.
  • Communicate complex information sensitively and effectively to service users, families, and professionals.
  • Submit funding applications and represent cases to commissioners or panels.
  • Maintain accurate records and meet performance targets in line with quality standards.
  • Uphold professional standards, values, and compliance with Social Work England registration.
Qualifications
  • Qualified Social Worker (Degree in Social Work, DipSW or equivalent).
  • Registered with Social Work England.
  • Completion of ASYE plus at least 2 years' post-qualifying experience.
  • Strong knowledge of adult social care legislation, including Care Act 2014, Mental Capacity Act 2005, Mental Health Act 1983, Equality Act 2010, and Human Rights Act 1998.
  • Proven experience working with vulnerable adults and carers, including safeguarding enquiries and statutory duties.
  • Experience of multi-disciplinary working and arranging support plans/packages of care.
  • Excellent communication, organisational, and IT skills.
  • Ability to travel across the Borough and work flexibly, including occasional evenings and weekends.
Desirable
  • Experience preparing statements/reports for court proceedings.
  • Best Interest Assessor training.
