About the role:

Lancashire is seeking an experienced and motivated Qualified Social Worker to join our Adult Services – Health and Wellbeing team. The successful candidate will support adults with complex needs, working collaboratively with partners across health and social care.

A strong and demonstrable understanding of NHS Continuing Healthcare (CHC) is essential for this role, including assessment processes, Decision Support Tools, and effective multi-disciplinary working.

Key Responsibilities:
  • Undertake strengths-based assessments and develop person-centred support plans.
  • Lead and contribute to NHS Continuing Healthcare assessments, reviews, and multi-disciplinary team meetings.
  • Work collaboratively with health professionals, service users, carers, and external agencies.
  • Manage a varied caseload in line with statutory duties under the Care Act 2014.
  • Ensure accurate, timely recording and adherence to professional standards.
  • Promote independence, wellbeing, and safeguarding across all service areas.
