About the role
  • Act as a key liaison between prison in‑reach teams and community drug / alcohol services to support continuity of care, shared case management, and coordinated risk reduction planning.
  • Carry out prison‑based screenings, initial assessments, and risk assessments, developing recovery plans in partnership with service users prior to release.
  • Provide harm reduction education, overdose prevention, and relapse management planning with service users in custody and following release into the community.
  • Deliver and promote Naloxone provision pre‑ and post‑release, and train service users and peers in its use where appropriate.
  • Offer “through‑the‑gate” support including gate pickups where appropriate and facilitate access to community recovery services, housing, and health appointments upon release.
  • Conduct home visits, hostel visits, and community outreach as needed to maintain engagement, particularly with high‑risk individuals and those at risk of disengagement.
  • Work collaboratively with police, probation, mental health, housing, and social care partners to address the complex needs of high‑risk service users and promote recovery and rehabilitation.

  • Approved for Restricted Institutional Access: The successful candidate will need to complete an enhanced DBS check and obtain HMPPS security clearance.
