A prominent healthcare provider in Mountain Ash is seeking a qualified pharmacist to manage and deliver primary care medicines management services. The role involves leading multidisciplinary teams, supporting royalty in medicine optimization strategies, and contributing to key operational programmes. Candidates must possess a vocational Bachelor's or Master's in Pharmacy and relevant registration with the General Pharmaceutical Council. The position offers a part-time permanent contract with competitive salary.
The post holder will be responsible for managing, leading, developing and delivering primary care medicines management services for Cwm Taf Morgannwg University Health Board (CTMUHB); developing the medicines management service model and delivery plan for the transformation multidisciplinary teams (MDT) within primary care.
The post holder will support the Chief Pharmacist to develop and implement the UHB medicines optimisation strategy and plan by ensuring our Cluster Pharmacy Teams are aware of, and aligned with the medicines management plans for safe, evidence based and cost-effective primary care prescribing.
The post holder will be the medicines management lead on key strategic, operational, transformational programmes and initiatives within primary care requiring medicines management input.
