Job Title: Primary Care Physician (Internal Medicine / Family Medicine) (MD/DO)

Position Summary:
We are seeking a dedicated Primary Care Physician to provide comprehensive, patient-centered care across the lifespan. The physician will diagnose, treat, and manage acute and chronic conditions, emphasize preventive care, and coordinate services to support long-term health and wellness.

Key Responsibilities:

  • Provide comprehensive primary care services, including preventive care, health maintenance, and management of acute and chronic conditions.
  • Conduct patient evaluations, physical examinations, and diagnostic assessments.
  • Develop and implement individualized treatment plans and coordinate care with specialists as needed.
  • Provide patient education on disease prevention, lifestyle modification, and chronic disease management.
  • Manage continuity of care through follow-up visits and care coordination.
  • Maintain accurate, timely, and compliant medical documentation.
  • Participate in quality improvement initiatives, population health efforts, and continuing medical education.

Qualifications:

  • Doctor of Medicine (MD) or Doctor of Osteopathy (DO) degree from an accredited institution.
  • Completion of an accredited residency in Internal Medicine or Family Medicine.
  • Board certification or eligibility in Internal Medicine or Family Medicine.
  • Active medical license to practice in the state of employment.
  • Strong clinical judgment, communication skills, and commitment to patient-centered care.

Work Environment:

  • Outpatient clinic setting with potential for limited inpatient responsibilities depending on practice model.
  • Team-based, collaborative environment with support from nursing, care coordination, and ancillary services.
