Responsibilities

• Carry out structured Medication reviews with patients.
• Provide prescribing advice and support to the practice.
• Implement change in prescribing behavior when needed.
• Undertake medication reviews and therapeutic specific medication reviews.
• Support practice staff on all medication issues.
