Job summary

A Community Healthcare Provider in Gateshead is seeking a compassionate and committed full-time nurse to join their team. The successful candidate will deliver high-quality care to older and vulnerable patients, work collaboratively within a multi-disciplinary team, and uphold high standards in nursing practices. Overview

Gateshead Inner West PCN are seeking a caring, compassionate and committed nurse who is self-motivated, hard-working, and patient focused to join our team.

The closing date is 17 October 2025

This role is offered on a full-time basis, for 37.5 hours per week, Monday - Friday. Pay scale: Equivalent to NHS Band 5 (non-Agenda for Change contract).

Main duties of the job
  • To support the organisation and facilitation of clinical care, in a holistic and proactive manner, to patients registered within our PCN.
  • To maintain a high professional standard of nursing care, in adherence to all PCN protocols, policies and guidelines, as well as to provide nursing assistance to the GP's and other members of the primary healthcare team
  • To demonstrate flexibility, in line with the present atmosphere for change within the health service delivery.
  • To work within your own level of competency, and expanding the role in accordance with PCN requirements and own scope of practice

Based in the Inner West of Gateshead our Primary Care Network serves over 45,000 patients across our five member Practices of: Whickham Medical Cottage, Chainbridge Medical Partnership, Glenpark Medical Practice, Teams Medical Practice & Sunniside Surgery.

We provide integrated services to our local population. Aiming to deliver more personalised, coordinated health and social care for people close to home.

The successful candidate will work within the community of our locality across all five sites.

Job responsibilities

Clinical Duties

  • Delivering highquality, compassionate care for older & vulnerable people living at home either alone or with their family so that they receive proactive care that steers them away from avoidable unscheduled care.
  • To provide a continuously improving service in line with the clinical governance framework of The Primary Care Network Practices as well as the local and national drivers of older peoples services.
  • The post holder will work as a member of the PCN complex care team but will need to be flexible to the needs of given practices within our network.
  • Undertake clinical tasks when required.
  • Play an integral role in the delivery of vaccination campaigns.
  • To assess, plan, implement and evaluate care plans for patients; promoting proactive independence and autonomy; working within a multi-disciplinary team.
  • To work in collaboration with other professionals to deliver and ensure care is delivered appropriately.
  • To demonstrate clinical effectiveness of service delivery by use of evidence-based practice and outcome measures.
  • To have the skills to take a history, assess, examine, diagnose, and develop a management plan.
  • To recognise, assess and manage risk across the immediate and wider working environment and make appropriate decisions autonomously ensuring statutory requirements are met.
  • To be responsible for patient safety through knowledge of systems, legal requirements and understanding of litigation.
  • To communicate effectively verbally and in written form in the exchange of highly complex, sensitive, or contentious information in demanding situations using de-escalation, mediation, resolution, and professional Duty of Candour.
  • To evaluate care, taking appropriate action leading to improvement in quality standards through clinical audit, root cause analysis and dealing with complaints.

  • Registered as a General Nurse with NMC
  • Nursing Experience within General Practice and/or Primary Care

  • Competent in the use of Office, Outlook & EMIS
  • Effective time management (planning & organising)
  • Ability to work as a team member and autonomously.
