Overview

Join our dynamic team at Meddygfa Glan Cynon Surgery in Mountain Ash, South Wales. We are seeking an enthusiastic Clinical Pharmacist to lead medicines optimisation and quality improvement, ideally from a primary care background. This part-time role offers 2 to 3 days per week with a negotiable salary within band 7 or 8 based on experience. You will collaborate within a multidisciplinary team in a patient-facing role to deliver excellent patient care.

Main duties of the job
  • Conduct clinical medication reviews for patients with multiple long-term conditions, ensuring a patient-centred approach.
  • Participate in multidisciplinary clinical meetings within the practice and Primary Care Cluster, LMC and Prescribing team within CTMUHB.
  • Manage repeat prescription requests and queries within your scope of practice, ensuring patients have appropriate monitoring and safety checks in place.
  • Action changes to medicines in response to MHRA alerts, product withdrawals, supply shortages, and relevant local or national guidance.
  • Maintain accurate and thorough records of all patient interactions using appropriate clinical templates and coding in line with practice protocols.
  • Take a proactive role in clinical audit and quality improvement initiatives, implementing recommendations to enhance patient care.
  • Provide patients with clear, independent information regarding their medications and any changes to their prescriptions, referring to other healthcare professionals for additional support when necessary.
  • Monitor prescribing patterns within the practice against local health board guidance, and make recommendations or action changes within your scope of practice based on findings.

Person Specification

Qualifications

  • Completion of an undergraduate degree in pharmacy and registration with the General Pharmaceutical Council.
  • Independent Prescriber or currently working towards qualification.

Experience

  • Minimum of 2 years experience as a pharmacist. Experience with common acute and long-term conditions likely to be seen in general practice. May hold or be working towards an independent prescribing qualification. Able to recognise priorities, refer to seniors or GPs when appropriate, and follow legal, ethical, professional and organisational policies and codes of conduct. Involves patients in decisions about prescribed medicines and supports adherence as per NICE guidelines.
  • Understanding of the inner workings of a primary care setting.
