Job Summary

A new opportunity has arisen at The Elms Surgery to join our family‑friendly, patient‑centred, forward‑thinking practice based in the centre of Watford. We are looking for an enthusiastic, experienced, proactive, Practice‑Based, lead Prescribing Clinical Pharmacist. Main Responsibilities
  • Support the practice team and its patients
  • Perform medication reviews
  • Re‑authorise repeat prescriptions
  • Process hospital discharge letters
  • Support and manage long‑term condition clinics
  • Conduct regular audits
  • Support QOF and ECF targets
  • Steer the development of the prescribing team
Key Responsibilities
  • Fulfil locally defined outcomes, optimising medicines for patients on complex regimes.
  • Improve medicine management to reduce adverse effects and maximise clinical outcomes through patient engagement.
  • Reduce potential waste by synchronising medicines and identifying non‑adherent patients.
  • Ensure cost‑effective prescribing by fully utilising resources and implementing changes to current prescribing.
  • Undertake and record reviews of discharge/clinical letters, ensuring accurate entry in the clinical system and communicating changes to patients.
  • Conduct medication reviews with direct patient contact for those on complex regimes or high risk of harm, including care‑home residents.
  • Liaise proactively with clinical staff to offer this service to eligible patients.
  • Record interventions using the provided template, detailing actions that may have prevented admissions.
  • Consider tapering, withdrawing or discontinuing medications to reduce over‑prescribing and adverse effects during reviews.
  • Undertake regular medication‑related audits and implement identified actions.
  • Communicate with staff at all levels within and outside the practice.
  • Effectively communicate complex medication issues to clinicians and to patients/carers with discretion and understanding.
Leadership
  • Act as a support resource for patients with complex pharmaceutical needs, liaising with GPs, PAs, care coordinators and social care staff.
  • Serve as a practice champion for medicines optimisation.
  • Promote standards for the quality use of medicines to improve patient outcomes.
  • Collaborate with care coordinators to support practice, local and national targets.
  • Develop and support a prescribing team within the practice.
Education, Training and Development
  • Act as an in‑house resource expert on medicines optimisation.
  • Participate in clinical audit and lead audits of medicines‑related activities, e.g., prescribing.
  • Develop and deliver appropriate training for non‑clinical staff and patients on medicines use, including direct patient contact and therapeutic updates.
  • Keep abreast of current drug safety advice and alerts; circulate relevant audits and guidance to GPs, pharmacists and other personnel in a timely manner.
  • Agree, implement and monitor actions required for the medicines management annual plan as requested by the ICB.
  • Attend prescribing meetings and participate in biannual medicines optimisation practice meetings to support ECF targets.
  • Contribute to the development and implementation of care pathways involving medicines, aligned with national and local guidelines and prescribing policies.
Clinical Duties
  • Maintain professional accreditation and CPD as required by the GPhC.
  • Prescribe acute and repeat medication for patient need within areas of competency.
Person Specification
Qualifications
  • Essential: Bachelor's Degree in Pharmacy.
  • Essential: GPhC registration holder who has completed a CPPE pathway (e.g., PCPEP or GPPTP).
  • Essential: Independent prescribing qualification.
  • Desirable: MPharm degree.
Experience
  • Essential: More than 2 years previous experience in a GP practice.
