We’re looking for a dedicated Social Worker to join our Transitions Team, supporting young people with Learning Disabilities and Autism as they move into adulthood.

This is a fantastic opportunity if you have experience supporting young people with Learning Disabilities and Autism through the transition into adulthood. You’ll be making a real difference at one of the most important stages in their lives.

Responsibilities
  • Support young people with Learning Disabilities and Autism through the transition into adulthood as part of the Transitions Team.
Qualifications
  • Social Work qualification
  • Registration with SWE
  • Experience in Transitions, CoP DoLS, and/or supporting young people with LD & Autism
