Boundary House is a well-establishedtraining practice with a list size of circa 11,400 in Sale.
We use EMIS web, Docman and AskmyGPonline platform for patient communication.

The team at BHMC comprises of 3 GP Partners, 6 Salaried GPs,1 Advanced Clinical Practitioner, 2 Pharmacists, and 2 Nurses, I trainee Nurse, 2Health Care Assistants along with non-clinical team of 17 staff managed by the Practiceand Office Manager.
Our practices have been rated as Good by CQC and we takepride in delivering quality care to our patients and have a strong training andpersonal development ethos.
We are using EMIS Web and DOCMAN as our clinical systemalong with Ask My GP to provide digital platform to our patients.
Providing assessment, screening and treatment services and health education advice
Organise and co-ordinate the provision of nursing services for the practice, to include the delegation of duties to other practice nursing/ HCA staff as appropriate
Provide nursing treatments to patients in participation with general practitioners or independently, meeting agreed protocols
Provide general and specific health screenings to practice patients (within agreed protocols) with referral to general practitioners as necessary
Advise patients on general health care and minor ailments with referral to GPs as necessary
Clinical areas examples (where appropriately trained)
Chronic Disease Management Lead on Diabetes
Diabetic review and management /education
Nurse Prescriber
Asthma/Peak Flow Readings/Spirometry if appropriate
Cytology
Travel vaccinations (NHS covered only)
Child immunisations
Routine immunisation
Requesting pathology tests, for example urine culture, swabs
Infection Control involvement, with administration support
Supplies and equipment treatment room and other areas
Ensure the maintenance of equipment and stock levels relating to patient care
Pathological specimens and investigatory procedures
Undertake the collection, storage and despatch of pathological specimens including intravenous blood samples, swabs, smears etc. and ensure the cleanliness of any equipment so used. Perform any investigatory procedures requested by the GPs.

Job summary:
To provide high quality patient care and treatment. Participate in settings standards for care and auditing clinical outcomes. Promoting health promotion programmes for patient, carers and facilitate screening clinics.
Provide nursing care in a variety of settings by holistic assessment, implementation and evaluation of programmes of care as appropriate.

Attend any external training course thought to be appropriate to the role.
