A local medical practice in Rotherham seeks a Practice Nurse to join their team. The role involves providing top-notch nursing care, engaging in chronic disease management, and supporting GPs. Ideal candidates will be RGN/NMC registered and experienced in working within a nursing team. This position also requires knowledge in cervical smears and QOF systems. Parkgate Medical Centre is a friendly practice. We have 3 GP Partners, a Salaried GP and a vast clinical team supported by our knowledgeable administrative team. We are also a training Practice with Registrars, F2s and medical students.
The Practice Nurse position aims to provide high quality nursing care to meet the needs of the practice population which includes treatment, care, screening, preventative care and patient education.
Duties will include:
Provide patient health promotion/disease prevention education and counselling focusing on health behaviour change.
Liaise with community nursing colleagues and secondary care to continue the provision of nursing care.
Provide support to the GPs.
To provide support in all practice services i.e. PMS, LES, DES and other incentive schemes.
Manage and treat long‑term conditions in line with national and local policies and practice need.
Maintain accurate patient records, entering onto the practice system using appropriate agreed codes, using agreed standards of data entry and using agreed practice templates.
Adhere to all Practice policies and procedures.
Collate the necessary data collection for QOF and the Quality Contract.
Participate in Infection Control procedures and maintain the cold‑chain procedure.
