Job description
Overview

We are looking for an experienced and motivated Practice Nurse to join our friendly primary care team on a 12‑month maternity cover contract. The role is suitable for a Band 6 or Band 7 nurse, depending on experience and scope of practice. You will provide high‑quality, patient‑centred nursing care and contribute to the effective management of long‑term conditions within the practice.

Main duties of the job
  • Provide high‑quality nursing care to patients across the practice population
  • Undertake chronic disease management clinics (for example asthma, COPD, diabetes, hypertension)
  • Deliver cervical cytology in line with national screening guidelines
  • Administer childhood and adult immunisations and vaccinations
  • Carry out health promotion and disease prevention activities
  • Manage long‑term conditions in accordance with evidence‑based protocols and QOF requirements
  • Support infection prevention and control standards within the practice
  • Maintain accurate and timely clinical records using the practice clinical system
  • Contribute to clinical audits, service development and quality improvement initiatives
  • Participate in policy development and review

Job details

Hours: 30–37.5 hours per week (negotiable)

Contract: Fixed‑term 12 months (maternity cover)

Location: Woolton House Medical Centre

Salary: Band 6 / 7

Key Responsibilities
  • Provide high‑quality nursing care to patients across the practice
  • Undertake chronic disease management clinics (e.g. asthma, COPD, diabetes, hypertension)
  • Deliver cervical cytology in line with national screening programmes
  • Administer childhood and adult immunisations and vaccinations
  • Support QOF and long‑term condition management
  • Maintain accurate and timely clinical records using the practice clinical system
  • Adhere to infection prevention and control policies

  • Maintain NMC registration and participate in revalidation
  • Take responsibility for maintaining and developing clinical competencies

Person Specification

Essential

  • Registered Nurse (Adult) with current NMC registration
  • Experience working as a Practice Nurse or within primary care
  • Competence in chronic disease management
  • Competence in immunisations and cervical cytology
  • Ability to work independently and as part of a multidisciplinary team
  • Good communication, organisational and IT skills

Desirable

  • Experience using GP clinical systems
  • Willingness to undertake further training
