MUST HAVE PREVIOUS PRACTICE NURSE EXPERIENCE

A position has arisen for an enthusiastic and motivated individual to undertake the role of Practice Nurse to work alongside our current practice nurse we are ideally looking for around 18 hours.The ideal candidate should be registered with the NMC, must be working or have had practice nurse experience and provide certification of training courses undertaken for the relevant post.

To offer a high standard of evidence based care to patients, to include health promotion, treatment, screening and disease management.Specific clinical areas include:National immunisation and vaccination scheduleTravel Immunisations and VaccinationsEssential hypertension/CKD/CHDCervical smearsUrinalysisChronic Disease ManagementHome Visits

We are an established, friendly and supportive GP Training Practice based in Rainhill Merseyside, we have a patient list size of around 9200, with a team of 2 GP partners,4 salaried GP's, 2 Practice Nurse's and Healthcare Assistant and an experienced Administration and Reception Team who offer a wide range of support. We use EMIS web clinical system along with Docman 10 and other clinical systems.We pride ourselves on delivering excellent care to our population.

Job responsibilities

This is not a comprehensive list but outlines min. requirementsProvide general nursing care to practice population who are referred or self-refer in the practice or in home setting including assessment, planning, delivery and evaluation of:Administer nebuliserSpirometry/respiratory testingCervical cytologyBlood pressure and other physiological measurementsHelp the practice develop and deliver specialist primary nurse led services including chronic disease management. Initiate the role of diagnosis for patients suspected to have a chronic disease e.g. diabetes, COPD, asthma and CHD, referring to other clinicians as appropriateManage programs of care for patients with chronic diseases by planning, providing and evaluating care under agreed guidelines and patient group directions. Maintain disease registers in liaison with administrative staff. Undertake annual review of patients understanding and ability to self-manage .Provide comprehensive travel health advice for patients and administering of travel vaccines Administer child and adult immunisations and vaccinations in accordance with national and local programs. Competent in anaphylaxis and resuscitation techniques. Ensuring safe storage, rotation and disposals of vaccines and drugs and that adequate stock levels are maintainedUndertake autonomous comprehensive assessment of patient need and develop care plan in accordance with local and national protocolsMake judgements on range complex issues requiring analysis, assessment and solutionDeliver care to specialist groups in accordance with practice protocols and QOF guidance, local and national guidanceProvide health screening to practice populationAutonomously refer to multidisciplinary team for referral/ treatment/ dischargeVisit patients in their homes to deliver primary nurse led servicesSafely use and manipulate equipment used in examination and treatmentsComputer literate with practice patient record system and electronic communication systemsSafely manage hazardous biological substances including blood/urine/vomit/faeces/vaginal secretionsCommunicate effectively with patients, relatives and carers regarding treatment and progress of patients, maintaining confidentiality of all patient informationManage issues arising from sensitive situations e.g. unwanted pregnancy, sexually transmitted infection, child protection issues mental healthAct as patient advocate/chaperoneAwareness and provision of evidence based carePhysically handle patients to assess/treat in emergency situationsTo provide home visits to patients.Health and safetyInfection Control lead
