A healthcare practice in Sussex is looking for a dedicated Practice Nurse to work 20-25 hours per week. KeyResponsibilities:

As a GP Training Practice and proud memberof the Greater Wealden PCN, we are committed to delivering exceptional care ina supportive and collaborative environment.
We are a busy,friendly, and expanding three-partner practice, currently caring for apopulation of approximately 16,000 patients across our dispensing sites. Tosupport our growing services, we are looking to recruit a dedicated PracticeNurse to join our experienced clinical team, working flexibly across oursites.
The followingare the core responsibilities of the practice nurse. There may be on occasiona requirement to carry out other tasks; this will be dependent on factorssuch as workload and staffing levels:
a.Develop,implement and embed health promotion and well-being programmes
b.Implementand evaluate individual treatment plans for chronic disease patients
c.Identify,manage and support patients at risk of developing long-term conditions, preventingadverse effects on patients health
d.Provideroutine nursing care to patients as required in accordance with clinical-basedevidence, NICE and the NSF
e.Requestpathology services as necessary
f.Processpathology results as required
g.Providechronic disease clinics, delivering patient care as necessary, referringpatients to secondary/specialist care as required
h.Maintainaccurate clinical records in conjunction with extant legislation
i.Ensureread codes are used effectively
j.Maintainchronic disease registers
k.Chaperonepatients where necessary
l.AssistGPs with minor surgery when required
m. Prioritise health issues and interveneappropriately
n.Supportthe team in dealing with clinical emergencies
o.Recognise,assess and refer patients presenting with mental health needs
p.Implementvaccination programmes for adults and children
q.Supportpatients in the use of their prescribed medicines or over-the-counter medicines(within own scope of practice)
r.Liaisewith external services/agencies to ensure the patient is supportedappropriately (vulnerable patients etc.)
s.Delegateclinical responsibilities appropriately (ensuring safe practice and that thetask is within the scope of practice of the individual)
t.Supportthe clinical team with all safeguarding matters, in accordance with local andnational policies
u.Understandpractice and local policies for substance abuse and addictive behaviour,referring patients appropriately
v.Deliveropportunistic health promotion where appropriate
