Job Summary

20.5 hours per week. Part‑time, permanent contract. The role offers flexible working, job share and flexible patterns. It is based at Abbeyrise, Tavistock, in a scenic location with free on‑site parking, hybrid pool cars and a digital working environment.

Main Duties

You will work within the Livewell South West Community Rehab teams, alongside occupational therapists, speech & language therapists, support workers and health & social care coordinators. You will provide physiotherapy services in the community, often travelling across Devon and West Devon and occasionally to Plymouth.

Responsibilities
  • Perform physiotherapeutic assessment of patients with diverse and complex physical, psychological, cognitive and behavioural conditions to provide accurate diagnosis and prognosis.
  • Formulate and deliver individualised physiotherapy treatment programmes, including manual therapy techniques, therapeutic handling, patient education, exercise (individual and group) and alternative options.
  • Appropriately select referrals for inclusion in a range of treatment programmes and conduct one‑to‑one sessions using advanced treatment options.
  • Undertake evidence‑based audit and research projects to further clinical practice.
  • Make recommendations to the Team Lead for potential changes in practice and service, and lead on the implementation of specific changes.
  • Supervise, educate and assess the performance of physiotherapy students. Work with universities to meet teaching standards.
  • Demonstrate highly developed dexterity, coordination and palpatory senses for assessment and manual treatment.
  • Use verbal and non‑verbal communication tools effectively with patients and carers, motivating cooperation and progressing rehabilitation programmes, especially with those having communication challenges.
  • Accept clinical responsibility for a designated caseload, organising work efficiently with clinical priorities and time use.
  • Participate in working parties developing policy changes within the community and ensure staff implement policy and service development changes.
  • Work with the Team Leader in developing strategic and operational management of the ward physiotherapy service.
  • Provide spontaneous and planned advice, teaching and instruction to patients, relatives, carers and other professionals, and promote understanding of physiotherapy aims.
  • Provide specialist advice and training to other MDT members regarding the management of patients.
  • Advise on and prescribe rehabilitation aids, equipment and recommend low‑level adaptations for patients referred to community therapy, in line with organisational policies.
  • Initiate, organise and attend multi‑disciplinary/multi‑agency case conferences, liaison meetings and patient reviews as necessary.
  • Assess capacity, gain valid informed consent and comply with confidentiality, child protection and vulnerable adult procedures.
  • All staff are expected to be able and willing to work across a seven‑day service.
