Job description
Overview

This exciting role is for a physiotherapist looking to develop their specialist skills further in community stroke rehabilitation. The applicant should be an autonomous practitioner with proven skills in caseload management and specialist knowledge of stroke rehabilitation. The role works in a friendly supportive team across BNSSG working with people in their homes and local community. There is lots of scope for training and development and working closely with the MDT. This role is fixed term for 6 months.

Responsibilities
  • To assess, plan, implement and monitor care, facilitating best practice and evidence-based, person‑centred care.
  • To accept clinical responsibility for a caseload of stroke patients with complex/highly complex needs and to organise this effectively and efficiently with regard to clinical priorities and use of time, including reorganisation of planned work at short notice to accommodate emerging priorities.
  • To facilitate patient empowerment by enabling people to manage their condition effectively and independently preventing complications and improving quality of life.
  • To work within defined specialist, department and national protocols/policies for stroke care such as the NICE clinical guidelines and National Stroke Strategy.
  • To be a resource and provide specialist advice, education and support to other health care personnel and people who have had a Stroke contribute to planning, delivering and evaluating programmes of education and development for Stroke care.
  • To be actively involved in monitoring and developing the Stroke service, to ensure patients receive sustainable high quality care that is effective and safe.
  • To select and conduct standardised cognitive and perceptual testing to identify areas affecting functional ability and indicate need for further assessment/treatment.
