Workingas part of our established medicines management team at the Practice, thepostholder will play a key role in supporting the delivery of high-quality,safe, and effective medicines management for our patients. You will work withthe support of a Clinical Pharmacist and Senior Pharmacy Technician alongsideGPs, ANPs and the Repeat Prescription Administration team to improveprescribing processes, enhance patient care, and ensure the effective use ofmedicines within the practice. You will also support the practice in providing medication reviews and achieving the Quality & Outcomes Framework targets.

Job responsibilities

Clinical Support:

  • Assist in medication reviews
  • Reconcile medicines following hospital discharge and outpatient clinic appointments.
  • Provide advice to clinicians on suitable formulary alternatives for out of stock medication, the Pharmacy First Consultation Service and guidance on medications which may be purchased over the counter.
  • Handle patient's daily prescription queries (face to face, online or by telephone)
  • Offer guidance on medication use
  • Support long-term condition management
  • Promote patient self-care

Prescribing Support:

  • Work with prescribers to ensure adherence to formulary guidelines
  • Carry out audits to improve prescribing safety, quality and cost effectiveness
  • Contribute to the effectiveness of our medicines management team by providing support and training as appropriate.

Administration & Documentation:

  • Maintain accurate records
  • Support compliance with CQC and NHS Regulations
  • Reviews ways of working to maximise income from PPA & QOF
  • Liaise with HCAs to manage appropriate stock levels for emergency use and practice procedures.
  • Support safe, effective and cost-efficient prescribing by reviewing and managing repeat prescriptions and reducing waste.
  • Ensure compliance with practice protocols and best practices.
  • Carry out medication-related audits in agreeance with the Clinical Pharmacist and Senior Pharmacy Technician.
  • Keep up to date with professional responsibilities and CPD requirements
  • Contribute to ongoing continuous improvement of our operations and patient safety

Person Specification
Qualifications
  • Registered with the General Pharmaceutical Council (GPhC) as a Pharmacy Technician.
  • Relevant Pharmacy qualification (E.G. BTECT/NVQ Level 3 in Pharmaceutical Science or equivalent)
Experience
  • Experience working as a Pharmacy Technician in a primary care, hospital, or community pharmacy setting.
  • Experience in medicines optimisation, medication reviews, and repeat prescription management.
  • Experience working within a GP practice or Primary Care Network (PCN).
  • Experience using GP clinical systems (e.g., EMIS).
  • Experience conducting medication audits and contributing to QOF/PPA income maximisation.

  • Excellent communication and interpersonal skills, with the ability to engage effectively with patients and healthcare professionals.
  • Strong organisational and time management skills.
  • Ability to work independently and collaboratively within a multidisciplinary team.

  • Understanding of Quality and Outcomes Framework (QOF) and its role in primary care.
  • Knowledge of high-risk medication monitoring and safety procedures.
  • Able to maintain confidentiality at all times.
