Main duties of the job

This role supports a pharmacist in the early years of their career or new to general practice to develop clinical and operational expertise across primary care and community pharmacy. As part of a multidisciplinary team of pharmacists, clinicians, technicians, and community pharmacy professionals, you\'ll help deliver safe, effective, and person-centred pharmaceutical care across general practice. You will be supported to develop skills in medicines optimisation and clinical consultation to ensure seamless patient care and continuity between settings.

About us

Amicus Health is a thriving practice with five sites located in the picturesque Mid Devon area covering Clare House, Bampton, Witheridge, Morchard Bishop and Cheriton Fitzpaine surgeries. We deliver excellent primary care across Mid Devon thanks to our multidisciplinary clinical team of GPs, pharmacists, ANP\'s, paramedics, practice nurses, HCAs and a strong administrative function.

Job responsibilities

Key responsibilities include:

Primary Care:

• Conduct structured medication reviews and medicines reconciliation at GP practices, during clinics, or following patient discharge from other care settings.
• Support long-term condition management (e.g. hypertension, cholesterol, diabetes, asthma).
• Provide advice and resolve medication queries by collaborating with GPs, nurses, and other prescribers.
• Contribute to quality improvement projects, audits (QOF/IIF), and prescription safety initiatives.
• Update and maintain accurate electronic prescribing and medication records (e.g.,EMIS, SystemOne).

Community Pharmacy:

• Deliver NHS and private pharmacy services, including dispensing, clinical checking, and patient counselling.
• Provide and record services such as blood pressure checks, vaccinations, and health promotion campaigns.
• Offer medicines advice to patients and support minor ailment management.
• Work collaboratively with the pharmacy team on service delivery and workflow management.
• Support Medicines Use Reviews and the NHS New Medicine Service (NMS).
• Actively participate in the Pharmacy First service, providing clinical assessments, advice, and treatment for eligible conditions in line with national and local service protocols.

Additional Duties:

• Support, supervise, or mentor pharmacy students, technicians, or foundation pharmacists as appropriate.
• Provide training and updates to colleagues and wider primary care teams on medicines and prescribing.
• Contribute to an inclusive workplace culture and uphold organisational values.
• Work across sites as and when requested.
