A local healthcare provider in Tonypandy is seeking a motivated individual to join their team. Responsibilities include managing clinical medication reviews and chronic disease management while providing support to the clinical team. Candidates must have prior primary care experience and GPC registration. The practice is community-focused and aims to provide high-quality healthcare services.
The Practice is looking for a motivated individual to join the team at De Winton Field Practice, the role will include managing the repeat re-authorisations , medicines reconciliation from Hospital discharge etc , undertaking medication reviews . Providing advice to the practice clinical team, involvement with QI initiatives as appropriate.
There will also be a requirement to support the team with chronic disease management, nursing home patients , shared care medication and DOAC reviews/ initiation.

De Winton Field Practice is a training practice with a current list size of 9300 patients. Current staff includes 4 GP partners, 4 salaried GPs , cluster pharmacist, a pharmacy technician , an established nursing team, onsite physio service and mental health nurse. We aim to provide high quality primary care services and are located in a refurbished Health Centre. The practice is located approximately 40 minutes from Cardiff and has good transport links.
