Summary

Key Responsibilities
  • Conduct clinical structured medication reviews.
  • Manage patients with common/minor/self-limiting ailments within your scope of practice.
  • Offer advice to the Prescription Admin Team and address patient inquiries.
  • Reconcile medicines following discharge, identifying and rectifying changes.
  • Set up systems for continuity of medicines supply to high-risk patients.
  • Refer patients to appropriate healthcare professionals for timely care.
  • Support repeat prescribing reauthorization and identify high-risk patient cohorts.
  • Contribute pharmaceutical advice for the development of new services.
  • Analyze and present medicines data to support decision-making.
  • Undertake clinical audits of prescribing and implement changes.
  • Monitor practice prescribing against local health policies and guidance.
  • Provide newsletters and bulletins on important prescribing messages.
  • Deliver education and training on therapeutics and medicines optimization.
  • Ensure compliance with CQC standards where medicines are involved.
  • Support public health campaigns and provide specialist knowledge.
