Qualifications

Are you an experienced and motivated Clinical Pharmacist (IP) ready to take the next step in your career? Tadcaster and Selby Rural (TaSR) PCN is looking for a passionate Clinical Pharmacist to join our dynamic PCN team.

You’ll play a key role in shaping high-quality medicines optimisation, providing quality medication reviews and medication reconciliations across our three well-established practices, supporting around 32,000 patients across Tadcaster Medical Centre, Sherburn Group Practice and South Milford Surgery.

You will work closely alongside Practices forming an integral part of the clinical teams whilst also working alongside or PCN Team as part of a PCN Hub. The role will sit alongside other Pharmacists and Pharmacy Technicians.

The right candidate should have a passion and drive for providing high quality patient centred care both in person and via telephone. A prescribing qualification is essential for the role. The post-holder will be required to enrol on the General Practice Pharmacist Training Pathway (CPPE) if not already completed.

Note: The role is eligible for a NEST pension only.

Main duties of the job

As a Clinical Pharmacist with TASR, you’ll be responsible for a variety of Practice based clinical activities:

• Conducting structured medication reviews and supporting patients with long‑term conditions.

• Actioning acute and repeat medication requests, out of stock issues, medication reconciliation and complex polypharmacy requests.

• Leading medicines optimisation and quality improvement projects (MHRA reports).

• Managing common and minor ailments, and offering telephone medicines advice as an Independent Prescriber.

• Supporting medicines management in care homes.

• Reviewing and actioning prescribing safety alerts.

• Contributing to QoF, DES, and IIF outcomes.

• Leading clinical audits, sharing insights and driving improvements.

• Supporting the day‑to‑day management and development of the pharmacy team, including appraisals and clinical supervision.

Job responsibilities

You will also need an open and flexible attitude towards working with a variety of patient needs as well as Practice needs. Good verbal and written consultations as well as strong time management abilities are also needed.

Person Specification
Experience
  • Experience dealing with a variety of different patients with different communication needs.
  • Experience conducting structured medication reviews with polypharmacy patients.
  • Experience in General Practice setting.
Qualifications
  • Completion of an undergraduate degree in Pharmacy and registration with the General Pharmaceutical Council, therefore demonstrating an understanding of and conforming to the relevant standards of Practice.
  • Experience within General Practice or PCN network setting.
  • Successful completion of the Postgraduate Diploma in Clinical Pharmacy.
  • Services via the CPPE General Practice Pharmacist Training Pathway.
  • Knowledge of SysmOne (clinical system).
