PCN Care Homes Advanced Clinical Practitioner

Newgate Primary Care Network (PCN)is seeking a compassionate and skilledAdvanced Clinical Practitioner (ACP)to join our dynamic team. This role is ideal for an experienced practitioner who thrives in a collaborative environment and is committed to delivering high-quality care tocare home residents and community-based patients with complex needs.

The successful candidate will work closely with care home teams and GP's, delivering proactive, personalised care that improves outcomes and supports continuity of care. The role offers a balance ofcommunity-based visits and on-site careat Newgate Medical Group

This is a permanent part-time position, working 6 sessions per week across 3 days, with the opportunity to shape patient care pathways and contribute to the development of innovative services as the PCN continues to grow and evolve.

Main duties of the job

As an Advanced Nurse Practitioner, you will:

Participate in Care Home multidisciplinary team (MDT) meetings and ward rounds.

Conduct clinical assessments for community-based patients.

Develop and manage treatment plans for both acute and chronic conditions.

Prescribe and review medications for therapeutic effectiveness.

Promote health education and support patients in adopting self-care strategies.

Collaborate with general practice teams and other PCN professionals to optimize patient outcomes

About us

NMG is a single Practice PCN with over 30,000 patients. We are responsible for 4 care homes. We are involved in new projects to change the delivery of Primary Care. We are a training practice, welcoming medical students and registrars. We have an extremely cohesive, welcoming, well-established and diverse team, who provide the care to our patients to include: -

4 pharmacists, 2 pharm techs, 4 ACP's, 1 ANP, 1 Physicians Associate , 8 care co-ordinators who are responsible and lead on vaccinations, home visits, care home reviews, health checks, cancer care, safeguarding, LD/SMI/SMD reviews, weight management, prescription & triage/access support.

Large reception team, IT team, secretarial team, HR team and Finance team to provide support.

Experienced nursing team, HCAs, phlebotomist and Diabetes specialist Advanced Practioner.

NMG is close to Clumber Park, A1, M1 & within easy commuting distance from Sheffield, Doncaster, Chesterfield, Newark & Mansfield. London less than 1.5 hours by train.

Job responsibilities

Participate in regular Care Home MDT meetings and Care Home ward rounds at our 4 care homes to assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients

Visit patients in the community for clinical assessments

Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly

Assess, diagnosis, plan, implement and evaluate interventions/treatments for patients with complex needs

Proactively identify, diagnose, and manage treatment plans for patients at risk of developing a long-term condition

Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan

Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs

Support patients and Care Home staff to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care

Assess, identify, and refer patients presenting with mental health needs in accordance with the NSF for Mental Health

Provide advanced, compassionate, and patient-centered palliative and end-of-life care, aligning with residents' preferences while supporting families and care staff with emotional and practical needs.

Person Specification
Experience
  • Evidence of practice across the four pillars of Advanced Practice: Clinical Practice, Leadership & Management, Education, and Research.
  • Demonstrable experience working at an advanced level with a job plan or portfolio that shows equity with peers at this level.
  • Experience of independent prescribing and conducting structured medication reviews.
  • Proven experience working in a primary care or community care setting, managing patients with complex needs.
  • Previous experience working as a practice nurse, paramedic or other relevant clinician within a primary care or community environment.
  • Experience of care home or older adult care, including proactive management and multidisciplinary team working.
Other
  • Must have a full driving licence and access to own vehicle.
Qualifications
  • Registration with an appropriate professional body:
  • HCPC for paramedics or other allied health professionals
  • Advanced Practice Qualification at post-graduate level (Masters degree or equivalent).
  • Independent Prescriber qualification relevant to professional registration.
  • Evidence of meeting revalidation requirements for the relevant professional body (e.g., NMC, HCPC, GPhC).
  • Digital Badge holder
