A leading NHS Trust in Coventry is looking for a Patient Flow Coordinator to enhance integrated team services. This role involves coordinating patient care, managing assessments, and facilitating discharge processes while ensuring effective communication with families and care homes. An exciting opportunity has arisen for a Patient Flow Coordinator to support the new and transformational One Coventry Integrated Team (OCIT) - a pioneering service that brings together health and social care professionals to deliver seamless, person-centred care. You will be part of an innovative team supporting reablement, hospital discharge, and admission avoidance across urgent and emergency care pathways. Local integrated care is one of the Trust’s fundamental purposes, as set out in our refreshed strategy for 2022-2030; ‘More than a hospital’. The Trust aims to improve the health and wellbeing of local populations through collaboration with partners across the health and social care system. The Patient Flow Coordinator role is key to the successful delivery of this exciting and transformational service aimed to improve the health and wellbeing of the Coventry population.
The Patient Flow Coordinator will work autonomously within OCIT demonstrating effective time management, coordinating a varied caseload in line with dynamic capacity and demand challenges, whilst advising and supporting the local integrated teams with appropriate service pathways. The successful candidate will have effective communication skills, a high level of administrative skills, an eye for detail with a focus on service coordination and data collection.
