Job description
Patient Care Coordinator - Medical Receptionist

We have full time and part time hours available.

Applicants must be able to travel between our main surgery BN1 6AG, and our branch surgeries BN2 9SL and BN2 4GJ.

This role will involve working closely with the Practice team including members of the Primary Care Network Team to support them administratively to deliver and provide safe and effective patient care. To be responsible for undertaking a wide range of reception duties and the provision of general support to the multidisciplinary team.

Main duties of the job

Duties can include but are not limited to, greeting and directing patients, effective use of the appointment system, booking appointments, processing of information and assisting patients as required. To act as the central point of contact for patients, the distribution of information, messages and enquiries for the clinical team, liaising with multidisciplinary team members and external agencies such as secondary care and community service providers.

You will be the friendly first point of access for patients, guiding them through the various patient pathways.

You will need to be dynamic and organised, with the ability to understand the complexities of the Practice systems and the wider healthcare system.

You will be responsible for undertaking a wide range of duties including administrative tasks as directed.

The post holder will ensure excellent communication skills, good record keeping, and have experience providing excellent customer service skills.

About us

We are a large practice, we operate across 3 sites our main surgery Stanford Medical Centre, and 2 branch surgeries; The Islingword Road surgery and The Cockcroft surgery.

We have a large GP, Nursing and Administrative team.

GP Team: We have 6 GP partners and 7 salaried GP's

Nursing Team: We have 5 Nurses, a Health Care Assistant, Phlebotomist

Pharmacy Team: We have 3 Pharmacists and a pharmacy technician

Other Clinical staff: Mental Health Practitioner, 3 First Contact Practitioner's

Job responsibilities

Patient Care Coordinator Primary Responsibilities:

To act as a key point of communication between the Practice and GP Practices within the Primary Care Network and other healthcare organisations.

Working to the standards and founding principles set by the Practice.

Provide caring, un-biased and timely support to patients and other individuals accessing the Practice.

Input data into the patients healthcare record as necessary.

Answer incoming phone calls, transferring calls or dealing with the callers request appropriately.

Maintaining and monitoring the practice appointment system.

Keeping patients informed of their appointments should they be delayed or cancelled. Process personal, telephone and e-requests for appointments.

Answer incoming phone calls, transferring calls or dealing with the callers request appropriately.

Signposting - receive, assist and direct patients in accessing the appropriate service or healthcare professional.

Initiating contact with and responding to, requests from patients, team members and external agencies. Read code data on EMIS Web.

Photocopy documentation as required.

Data entry of new and temporary registrations and relevant patient information as required.

Input data into the patients healthcare records as necessary.

Direct requests for information i.e. SAR, insurance/solicitors letters and DVLA forms to the administrative team

Manage all queries as necessary in an efficient manner.

Processing incoming mail and electronic mail as appropriate.

Carry out system searches as requested.

Maintain a clean, tidy, effective working area at all times.

Monitor and maintain the reception area and notice boards.

Support all clinical staff with general tasks as requested.

Maintaining the efficient patient records system, including filing and following requests.
