The Paramedic will work with our GPsto decide on and implement appropriate patient management. You will also workclosely with our local community nurses and other members of our growingintegrated care teams. It is envisaged that they will also be part of amultidisciplinary team providing urgent care within practices, working withGPs, nurse practitioners and will see patients both in practice and on homevisits.

Marlow Medical Group offers a supportive environment where you will autonomously assess, diagnose, treat, and evaluate a wide variety of patients and pathologies in line with local and national guidelines.The post holder is an experienced nurse who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking right through to diagnosis, treatment, and evaluation of care.The post holder will be under the supervision of a senior clinician, providing a range of services, such as triage, telephone and face-to-face appointments, clinical decision-making, referring patients appropriately and interpreting results.Asking advice or learning is very much encouraged and career development is supported.

Job responsibilities

Topractice as an emergency care practitioner and demonstrate advanced clinicalcompetence and knowledge above those associated with conventional nursingroles.

Assess,plan, treat and refer patients appropriately, carrying out consultations in thesurgery or the community as an autonomous practitioner.

Usingown clinical judgement to diagnose and treat patients either face to faceconsultation, clinical telephone triage and other mediums.

Makingappropriate referrals to primary, secondary and social services care, followingpractice (and/or locally agreed) pathways and protocols.

Deliverhigh quality clinical care in a timely manner to patients.

Supply,administer and prescribe medicines as indicated (and appropriate) to addresspatient need, according to medicines management policy, Patient GroupDirections (PGDs), NMC standards or referring to registered GP as necessary.

Torecognise the need for and initiate where appropriate relevant diagnosticinvestigations, using their skills in this area to take the correct course ofaction.

Understandinglocal safeguarding referral process and be aware of appropriate referralmechanisms for children and vulnerable adults

Ensurecomplete and accurate documentation of every patient contact, including advicegiven. Records must be accurate and reflect the clinical activity in accordancewith NMC guidelines on clinical record keeping.

Minorillness assessment, diagnosis and treatment with access to seek advice, reviewand onward referral within clinical teams at Marlow medical Group.

Todevelop care plans for patients when required (as developed and supervised withthe Partners)

Beinvolved with continuity of care of our patients, e.g. end of life care,chronic disease management.

Participationin administrative systems in the Practice.

Attendand participate in Practice meetings when required.
