As part of a dedicated and supportive multidisciplinary team (MDT), the post holder will play a key role in delivering high‑quality, family‑centred care to children and young people living with diabetes. The role involves working closely with paediatricians, dietitians, psychologists and specialist nurses to ensure continuity of care and promote best practice in diabetes management. The post holder will contribute to ongoing service development, audit and education, and will be encouraged to develop their knowledge and skills in line with the national Paediatric Diabetes Best Practice Tariff.
This position is ideal for a nurse who is passionate about improving outcomes for children and young people, keen to develop specialist skills in paediatric diabetes and eager to progress within a supportive and experienced team.
Responsibilities
- Support the delivery of high‑quality, evidence‑based care to children and young people with diabetes in both outpatient and community settings.
- Provide education and practical guidance to children, families and schools on diabetes management, including insulin administration, glucose monitoring, diet and lifestyle.
- Work collaboratively within the multidisciplinary team (MDT) – including paediatricians, dietitians, psychologists and specialist nurses – to ensure coordinated, holistic care.
- Participate in diabetes clinics, home and school visits and telephone or virtual consultations to support families in achieving optimal diabetes control.
- Promote self‑management and independence for children and young people in line with developmental stages and individual needs.
- Contribute to the collection of data and audit activity to support service development and compliance with the National Paediatric Diabetes Audit and Best Practice Tariff.
- Engage in ongoing professional development, including attendance at relevant study days and completion of further qualifications where appropriate, to progress towards a Band 6 Specialist Nurse role.
- Provide support, guidance and education to colleagues and students to promote awareness and understanding of paediatric diabetes across the wider paediatric service.
- Participate in the on‑call or emergency advice rota (if applicable) following appropriate training and competency assessment.
- Co‑ordinate a clinical caseload as directed by the Lead Paediatrician.
- Work autonomously and be accountable for own actions to assist children/young people and their families in maintaining independence, including regulation of insulin therapy.
- Educate diabetic children, young people and their families/carers in the principles of diabetic management, including insulin delivery systems, oral therapies and dose adjustment.
- Be aware of BPT income associated with specialist nurse services and manage resources associated with the best practice tariff for paediatric diabetes care.
- Undertake a comprehensive clinical assessment of children and young people using an appropriate model and level of clinical judgement.
- Maintain accurate and appropriate standards of documentation.
- Actively participate in the collection and collation of data for audit and research.
- Participate in nurse‑led paediatric diabetes clinics.
- Deliver education/training to schools, including care planning for pupils with diabetes.
- Participate in education/teaching programmes to meet the needs of staff groups.
- Support and facilitate the smooth transition from paediatric to adult services.
