Job summary

A leading charity in mental health and social support in Bradford is seeking an Outreach Worker to provide essential housing support and street outreach services. The role involves helping individuals experiencing homelessness and assisting them towards independent living, ensuring that they achieve their personal recovery goals. Candidates should have NVQ Level 3 in Health & Social Care or equivalent and experience working with vulnerable populations. A comprehensive benefits package is provided, along with a fixed-term contract of 36 months.

Qualifications

  • Understanding of issues that people face in their everyday lives.
  • Ability to maintain and work within professional boundaries.
  • Knowledge of completing service user assessments including comprehensive and risk assessments.
  • Experience of supporting vulnerable people.

Working Hours: 37 hours per week, Monday-Friday

Contract Type: Fixed Term (36 months)

Salary: £25,110 - £32,090 per annum

About the Role

As an Outreach Worker, you will work as part of a wider team to provide housing support and street outreach services, with the aim to accommodate those who are rough sleeping and support those who are vulnerably housed. To deliver a support package to help clients to live independently and progress towards sustained housing recovery.

What You’ll Do
  • Specialising in delivery of outreach services for adults who are currently sleeping rough, or at risk of doing so.
  • Supporting clients with obtaining suitable accommodation and support tenancy sustainment.
  • Undertaking outreach sessions, including referral agencies and other stakeholders to publicise the service, receive referrals and give signposting advice, and attending relevant multi‑agency meetings as required.
  • Undertaking regular needs assessments and developing comprehensive support/risk plans to identify how individual service user needs will be met in order to help each user live independently within the community and achieve sustained outcomes.
  • Supporting Service Users to live independently within the community and achieve sustained recovery by supporting them to achieve economic wellbeing, participate in education, training or other learning activities, develop or sustain a healthy and safe lifestyle and make a positive contribution to society.
  • Recording case notes, inputting information onto the database promptly and maintaining any other relevant Management Information Systems as required in order to ensure effective case management and assist monitoring and evaluation of service delivery.
  • Liaising with statutory and other appropriate agencies / networks / forums to contribute to the development of a comprehensive needs and risk assessment for each client and general service provision.
To Succeed in This Role, You’ll Need
  • NVQ Level 3 in Health & Social Care (or equivalent relevant subject)
  • An understanding of issues that people face in their everyday lives
  • Ability to maintain and work within professional boundaries
  • Knowledge of completing service user assessments including comprehensive and risk assessments
  • Experience of supporting vulnerable people
  • Knowledge of safeguarding adult and child protection policies
