THE EMPLOYER IS: BALLYMAGROARTY HAZELBANK COMMUNITY PARTNERSHIP. To provide practical support, guidance, and advocacy to individuals and communities facing various challenges, helping them access essential services, develop life skills, and improve their wellbeing and independence.

Responsibilities
  • Conduct assessments to identify individual needs, goals, and challenges.
  • Develop and implement personalised support plans and interventions.
  • Provide information, referrals, and signposting to community resources such as housing, healthcare, employment, and social services.
  • Engage with vulnerable groups, building rapport and trust to effectively deliver support.
  • Organise and facilitate outreach sessions, community events, and educational workshops.
  • Collaborate closely with local agencies, community organisations, and other stakeholders.
  • Advocate for clients' rights and ensure they receive necessary assistance.
  • Maintain accurate records, update client files, and produce reports.
  • Promote awareness on relevant issues within the community through talks, campaigns, or school visits.
  • Uphold safeguarding policies to protect vulnerable individuals.
  • Work as part of a multidisciplinary team, attending meetings and training as required.
