A charitable organization in Greater London is seeking an Outreach Health Navigator to engage with food bank users and vulnerable communities. This part-time role involves providing advice and support to improve access to health services, contributing to project evaluation, and making a real impact in addressing health disparities. The ideal candidate should have experience in community health outreach and strong communication skills to effectively assist diverse populations.
Community Links health team is embarking on a new chapter with funding secured to deliver an innovative 12-month pilot aimed at tackling health inequalities across London. As an Outreach Health Navigator you will play a pivotal role in the Community Health Bank project engaging with food bank users and other vulnerable communities in Newham to improve access to preventative health services and address disparities in health outcomes.
This is a frontline role where your engagement can make a real difference. You will provide advice, guidance and support to help people overcome barriers to healthcare while also contributing to the monitoring and evaluation of the project alongside the Project Coordinator and Programme Manager.
