Overview

An exciting opportunity has arisen for a Senior Community Occupational Therapist to join a new service in the Virtual Ward team. Virtual wards are a national initiative defined as a safe and efficient alternative to NHS bedded care enabled by technology (NHS England and NHS Improvement). Nottinghamshire Integrated Care System aims to deliver this through a Nottinghamshire wide provider collaborative, incorporating Sherwood Forest Hospitals, Nottingham University Hospitals, Doncaster & Bassetlaw Hospital, Nottinghamshire Healthcare and City Care to deliver an ambition of 400 - 500 beds across Nottinghamshire by Dec 2023. Virtual wards allow patients to receive care, remote monitoring and treatment they need in their own home, safely and conveniently rather than in hospital. They provide a significant opportunity to deliver care through collaboration of community services and acute hospitals as an alternative to acute admission and/or early discharge.

As a member of the Virtual Ward Multidisciplinary Team, the Community Occupational Therapist will support patient care to be delivered in the home, providing an alternative approach to receiving hospital care or clinical monitoring. The Virtual Ward position is based at various sites and delivers a joined-up approach across the Nottinghamshire Integrated Care System (ICS).

Responsibilities
  • Be pro-active in delivering anticipatory and responsive care, promoting independence, healthy lifestyle choices, and reducing health inequalities to improve overall quality of life in the local population.
  • Offer a comprehensive nursing/therapy service to the resident population of Nottinghamshire, providing a multi-disciplinary and multi-agency approach to maximise patient function, wellbeing and self-care planning.
  • Offer a comprehensive, needs-led therapy service to patients with rehabilitation needs on the virtual wards pathway, enabling care at home and preventing unnecessary hospital admission.
  • Work within the Virtual Ward team and in collaboration with system partners such as City Care and NUH as part of a supportive multidisciplinary clinical team focused on delivering excellent patient-centred care and innovation.
  • Provide care to patients either remotely or within a community setting, delivering high-quality holistic evidence-based care that is responsive, effective and efficient in line with the virtual ward model.
  • Demonstrate excellent communication and interpersonal skills, adaptability, reliability, resourcefulness, approachability, and the ability to establish effective working relationships in a range of settings.
  • Support patients within a community setting with timely assessment, treatment and effective discharge from the service.
  • Promote independence and prevent hospital admission or admission to care, supporting early discharge from hospital and enabling care at home.
  • Collaborate across organisational boundaries within a multidisciplinary framework, analyse and develop treatment plans while providing compassionate, realistic, and timely support to a diverse patient group and their families.
Requirements and Qualifications
  • Full UK driving licence and vehicle for business use is required for this post; reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.
  • DBS check: applicants are required to pay for their DBS check. Costs are deducted from salary after appointment (standard £26.40 or enhanced £54.40). The DBS application cost is deducted over the first two months of employment. You are encouraged to enrol for the DBS Update Service with an annual fee of £16 per year.
