Leeds Community Healthcare NHS Trust is currentlyrecruiting Band 5 Occupational Therapists to join our Core Therapy and ActiveRecovery Neighbourhood Teams. These roles provide an exciting and dynamicintroduction to community-based care, supporting patient flow through rapidresponse, admission prevention, and timely hospital discharge using a HomeFirst approach.

Our interventions are varied and impactful, coveringareas such as long-term condition management, falls prevention, urgent care toavoid unnecessary hospital admissions, and supporting patients to regainindependence at home. Youll develop a broad and diverse skillset while workingholistically with patients, using health coaching and self-managementapproaches to help them achieve their personal goals.

Main duties of the job

In thisrole, you'll have the opportunity to make a real difference in your patientslives while developing a broad range of clinical skills. You will work closelywith a wider multidisciplinary team, enhancing your knowledge and practicethrough regular clinical supervision, peer support, and continuing professionaldevelopment.

Whileyou will be fully supported by senior therapists, you will also takeresponsibility for managing your own caseloaddelivering holisticrehabilitation or appropriately delegating care. Youll be encouraged to workautonomously, develop your clinical reasoning, and strengthen yourcollaborative working skills, all of which will support your progression withinthe Trust.

Thisrole is part of a 7-day service, with working hours between 8am and 6pm.Weekend and unsocial hours attract enhanced pay. We are committed to supportinga positive work-life balance through our flexible working policy. You will bealigned to either a North or West Leeds hub, with the understanding that thisis a city-wide service and occasional cross-cover may be required. We willalways consider individual caring responsibilities when planning service needsand rotas.
