Key Responsibilities
  • Patient Engagement & Assessment: Provide clinical assessment, education, and ongoing support to patients within the virtual ward.
  • Monitoring & Interpretation: Oversee remote vital sign monitoring (e.g., heart rate, blood pressure, temperature, oxygen saturations) uploaded to our platform 3–4 times daily.
  • Clinical Decision Making: Use advanced clinical expertise to interpret trends, identify deterioration, and escalate care appropriately.
  • Condition-Specific Care: Manage patients with respiratory and cardiac conditions (e.g., asthma, COPD, pneumonia, heart failure) providing guidance on treatment and self-management.
  • Documentation: Maintain accurate, timely clinical records.
  • Collaboration: Work closely with a multidisciplinary team to deliver coordinated care.
Essential Requirements
  • Registered with the NMC
  • MSc in Advanced Practice or RCN Accredited (desirable)
  • Independent Prescriber
  • Significant experience in respiratory and/or cardiology (e.g., COPD, asthma, pneumonia, heart failure, post-MI)
  • Previous experience in a Virtual Ward, Hospital at Home, or acute community care setting
  • Confident in escalation pathways and managing deteriorating patients remotely
