Job description

The assessment of needs, planning, and delivery of care from assessment to discharge from services.
Implementing and evaluating care plans collaboratively with service users, carers, and other professionals, providing individualized care.
Working closely to empower and support their recovery journey.
The Band 6 Mental Health Nurse will support the caseload, oversee and manage risks by carrying out clinical risk assessments, and mitigate risks with a person-centered plan of care.
Ensuring that users can safely remain in the community or return home.
Taking responsibility for developing mental health knowledge and establishing links between community teams.
Advocating for people using the services.
