Mental Health Social Worker, Camden

Responsibilities

• Care coordination of complex cases under CPA
• Developing co-produced care plans with service users
• Risk assessment and management
• Safeguarding
• Holistic assessment of mental health and social care needs
• Conducting Care Act assessments
• Working with carers and wider support networks
• Partnership working with other statutory and voluntary organisations
• Liaison with inpatient units
• Accurate record keeping and report writing

Qualifications / Requirements

• Experience of working with adults with psychosis and complex needs
• Experience of Care Act assessments and reviews
• Organised with good time management and ability to prioritise workload
• Good interpersonal skills and ability to engage with service users from diverse backgrounds
• Ability to work as part of a multidisciplinary team
• Knowledge of relevant legislation, policy & procedure including Safeguarding, Mental Health Act, Care Act and Mental Capacity Act

Contract / Pay

£34.45 per hour
