Qualifications

• Experience working in a primary care environment.
• Experience in chronic disease management.
• Ability to record accurate clinical notes.

Responsibilities

• Conduct Structured Medication Reviews for high-risk patients.
• Supervise and appraise pharmacy technicians.
• Collaborate with PCN and practice managers.
