Overview

An exciting opportunity has arisen for a skilled and compassionate Band 6 Clinical Triage Nurse to join our Community Services team. This role is pivotal in ensuring patients receive timely access to the most appropriate care pathways through effective clinical triage and coordination. The post holder will be responsible for assessing referrals, prioritising clinical need, and liaising with multidisciplinary teams across community health services. You will support safe and efficient patient flow, enabling early intervention and reducing unnecessary hospital admissions. This role requires excellent clinical judgement, communication skills, and the ability to work both independently and collaboratively within a fast-paced environment. You will be part of a supportive team committed to delivering high-quality, person-centred care in the community. Our working hours are between 0800-2000 7 days a week and you will be required to work shifts within those times and days. Please be advised that sponsorship is not eligible for this post. Successful applicants will be required to provide an immunisation and vaccination report, from either their current UK occupational health provider or their UK GP.

Responsibilities
  • Triage referrals into community services using evidence-based clinical protocols.
  • Identify urgent care needs and escalate appropriately to ensure patient safety.
  • Liaise with GPs, hospital teams, social care, and other community providers to coordinate care.
  • Provide clinical advice and support to patients, carers, and professionals.
  • Contribute to service development, audit, and quality improvement initiatives.
