ModalityPartnership Lewisham Division has a full-time vacancy for INT Caseworker for 37.5hours per week on a fixed term contract for 2 years. The INTs are designed todeliver coordinated, community-based care and support. Were building a healthand care system thats not just sustainable but supports peoples physical andmental wellbeing. Main duties of the job

The INTCaseworker will play a central role in supporting the implementation anddelivery of Lewisham NHS 10 -Year Plan for integrated neighbourhood working.The role holder will focus on improving outcomes for residents by streamliningcare pathways, managing complex caseloads, conducting holistic assessments, andensuring individuals receive the right support through effective signpostingand coordination. Acting as the critical link between health, social care, andcommunity services, the INT Caseworker will ensure seamless, person-centredcare that tackles health inequalities.

The role is nota regular 9am-5pm job and is not a pure patient facing role; the job requires innovation,flexibility and commitment and the postholder will be required to work onquality improvement as outlined in the job specification and work resourcefullyas part of the team to ensure tasks are completed.

Knowledge
  • Previous experience in health, social care, or community coordination roles
  • Familiarity with face-to-face holistic assessment approaches and patient-centred care planning
  • Understanding of population health management and tackling health inequalities Competence in completing NHS Health Checks and Diabetes 8 Care Processes including blood pressure, weight, height and pulse checks
  • Knowledge of the Lewisham area and its health and care landscape
  • Experience with working with those with Learning Disabilities and SMI
Qualifications
  • Good general education with English and Maths to GCSE standard or equivalent Relevant NVQ 3 or equivalent qualification or experience
