Overview

Job summary: This post provides an opportunity for a trained nurse to join the South Yorkshire Regional Services ICC (Inherited Cardiac Conditions) service. The ICC provides a specialist service for familial conditions affecting the heart and major blood vessels, including Cardiomyopathies, Ion Channelopathies, Aortopathies, Neuromuscular conditions and Familial Hypercholesterolaemia.

The post holder will carry responsibility for assessing care needs, developing and evaluating care programmes, and setting standards of care. The ICC nurse provides specialist nursing care to newly referred patients and those under follow-up, including management of conditions and support for relatives at risk. This role includes coordinating investigations and interpreting results and delivering clinical screening where required. ICC patients can develop serious complications such as MI, heart failure, arrhythmia, vessel dissections and sudden death.

Responsibilities

Excellent communication and organisational skills are essential. The successful applicant will act as a source of expertise for patients, relatives and health professionals, and identify individual needs. Duties include:

  • Be a point of contact providing information, advice, screening information and supportive counselling for patients with inherited cardiovascular conditions.
  • Manage nurse-led clinics and maintain a diverse caseload, working autonomously and making independent decisions.
  • Perform nurse-led procedures (e.g., ajmaline provocation tests) and provide feedback to the lead Consultant Cardiologist.

If successful, you will act as a resource for other healthcare professionals, providing expert advice and information when required. The role involves collaboration with Cardiologists and liaising with Echocardiography; ECG; Neuromuscular services; Clinical Genetics; Surgeons; Coroners; Geneticists and Paediatric services as needed. The post also includes facilitating the transition of children aged 13+ to adult services and working across District General Hospital sites to support the ICC outreach service.

Qualifications
