Job description

As an Independent Living Facilitator, your role involves supporting young people with learning disabilities, autism, sensory needs, and physical disabilities to achieve their Education, Health, and Care Plan (EHCP) outcomes. You will provide person-centred, outcome-focused interventions in community and centre-based settings. Acting as a keyworker, you will facilitate tailored activities, build confidence, and enhance social inclusion and independence. You will also connect individuals with local networks, community services, and digital resources to boost their well-being and resilience.

We are looking for someone who is passionate and experienced in supporting individuals with complex needs, including behaviors of concern. You should be naturally skilled in facilitating activities that align with EHCP outcomes, and able to work flexibly in various settings. You must be someone comfortable with conducting risk assessments, providing personal care, and communicating professionally with colleagues, families, and external services.,
