A leading NHS organization seeks a Hospital Social Worker to support patients with neurological conditions at Salford Royal Hospital. An opportunity has arisen for a social worker to permanently join the specialist team based at Salford Royal Hospital, providing specialist social work support to patients who have suffered a stroke and have other long term Neurological Conditions. We invite experienced qualified social work practitioners (at least level 2 to level 3 post qualified) to apply for this permanent full-time post.
You will join an experienced and supportive Neurosciences Multi-Disciplinary Teams working across various wards and clinics and will gain valuable social work experience by working with this client group whose needs can be complex and challenging but very rewarding. You will provide social work assessments for the safe discharge of Salford residents, set up services as required, attend MDT and professional meetings while completing safeguarding investigations as required.
Ideally you will have some experience of working with this client group and of working in a hospital setting or similar.
