A leading healthcare staffing agency is seeking an experienced Hospital Social Worker for a 3-month contract at a London NHS Trust. The role involves working in the Emergency Department and helping to assess patients for discharge, focusing primarily on Camden residents. The position offers a competitive hourly rate of £32.16 and includes full support and induction. Ideal candidates should have a license and experience in social work.
We have a vacancy for an experienced hospital Social Worker to work at the Royal Free London NHS Foundation Trust (RFL) in the Emergency Department and Medical Assessment and Admission wards for 3 days a week (Friday/Saturday/Sunday). This post is to extend the Social Work support to the RFH for a period of 3 months. The purpose of the role is to work as a member of the Discharge Team, (medics, nurses and therapists) assessing patients who present at the weekend in the Emergency Department or who have been admitted to the wards. The aim is to prevent admission, re‑admission and reduce the length of stay for patients who are medically stable and can be discharged home with a care package or other support such as PACE (Post Acute Care Enablement). The Social Worker will be working primarily with Camden residents but may be requested to provide information and advice to out of borough patients. The person will be managed by the Camden Social Work Operations Manager and report to the TREAT (Triage Rapid Elderly Assessment Team) team leader at the weekend. To provide continuity and supervision, the post holder will on Fridays be based with the Camden Hospital Social Work Team at RFL. The successful applicant will be provided with induction and orientation in the first 2 weeks which will require working Monday to Friday.
£32.16 per hour.
