• Post registration experience in homecare pharmacy is advantageous.
• Experience with clinical investigations (e.g., near-miss investigations).
• Good mentoring and training capabilities.

Responsibilities

• Be the Responsible Pharmacist for prescription management.
• Conduct clinical screening and intervention.
• Dispense prescriptions and report errors timely.
• Order products and manage Patient Medication Records.
• Collaborate with clinical teams on patient adherence.
• Document adverse events and incidents.
• Provide clinical counselling for patients and support staff.
• Mentor pharmacy support staff.
