Job summary

A regional healthcare provider in Bournemouth seeks an Advocacy Care Coordinator to join their multidisciplinary team. The candidate will coordinate care for patients with liver disease and support initiatives in healthcare settings. Qualifications include NVQ level 3 in health/social care management and experience in coordinating care for vulnerable adults. Qualifications

• Experience of working with patients who have complex needs, liver disease and Hepatitis C.
• Understanding of Hospital based Hepatitis C procedures.
• Ability to document care assessments and progress.

Responsibilities

• Work with the Liver Nursing and Homeless Care team to advocate for patients.
• Coordinate care and engage patients with complex needs.
• Deliver Fibroscanning Clinics in clinic settings.

Education

NVQ level 3: health/social care management or equivalent

5 GCSEs including English & Maths
