Overview

We have an exciting opportunity for an experienced ANP / Nurse Practitioner to join our "Home Hospital" service designed to deliver coordinated healthcare to patients in the comfort of their homes, replicating the support and monitoring traditionally provided in a hospital ward. As an Advanced Nurse Practitioner (ANP), you will play a crucial role in this service, working remotely to engage with, assess, and support patients.

Responsibilities
  • Patient Engagement and Assessment: Provide education, support, and clinical assessment to patients within the virtual ward.
  • Vital Sign Monitoring: Oversee and monitor patient vital signs (e.g., heart rate, blood pressure, temperature) that are automatically uploaded to our platform 3-4 times daily.
  • Clinical Decision Making: Use clinical expertise to interpret vital signs, make informed decisions, and escalate care when necessary.
  • Patient Education: Educate patients on managing their conditions and using remote monitoring equipment effectively.
  • Documentation: Maintain accurate and timely records of patient interactions and clinical decisions.
  • Collaboration: Work closely with a multidisciplinary team to provide coordinated and comprehensive care.
Essential Requirements
  • Must be a registered NMC nurse
  • Must have an MSC in Advanced Practice or be RCN Accredited (desirable)
  • Must be an independent prescriber
  • Experienced in dealing with patients with respiratory conditions including asthma, COPD and pneumonia
